Answering no to a checklist item opens a tracked corrective action.

Why a note is required

If you answer no to a checklist item, you’re asked to describe what happened, in your own words. A no answer without an explanation isn’t accepted.

What happens next

The note opens a guided corrective action, walking whoever’s on shift through what to check and what to do next.

How it’s tracked to resolution

Every step is logged with who did it and when. Once complete, a manager or owner confirms it’s resolved — normally someone other than the person who did the fix.
